Consent form: survey

This research project is being conducted by Simon Lapierre, a professor at the University of Ottawa. If you have any questions, you can reach him by e-mail (simon.lapierre@uottawa.ca) or by telephone or text message (819-210-8850).

Invitation

If:
  • You are between 12 and 24 years old
  • You live in Quebec
  • You have experienced domestic violence by your father towards your mother
  • Your parents are separated
We invite you to take part in our research project, which gives a voice to children and teenagers who have experienced violence between their parents. In this project, we want to find out what children and teenagers think about the decisions that have been made following their parents' separation. We also want to know whether the people who made these decisions consulted them and asked them what they wanted. We're also interested in the relationship they have with their father since the separation.

You are invited to complete an online questionnaire, which will ask you questions about the decisions that were made following your parents' separation, and how these decisions were made. There will also be a few questions about your relationship with your father.
Participation

You'll need about 20 minutes to complete the questionnaire. You can access it by clicking on the link at the bottom of this page.


Benefits

By completing this questionnaire, you'll be able to give your opinion and tell us what you liked and disliked about the way these decisions were made. It's possible that this information will help other children or teenagers experiencing violence between their parents.

It's important for you to know that this won't change your personal or family situation.


Risks

Some of the questions may bring back sad or difficult memories. If this happens, you can always take a break. You can also decide at any time to stop participating in the research.

If you need support during or after completing the questionnaire, you'll also be given a list of organizations that can help you if you need it.

    Confidentiality and anonymity

    No one will be able to know that it was you who answered the survey.


    Data storage

    Questionnaires are stored on a password-protected computer. They will be securely destroyed 5 years after the end of the project.


    Voluntary participation

    Your participation is entirely voluntary. This means that you feel free to participate, and that you are free to refuse to answer certain questions or to change your mind along the way.
